As a service to faculty, ID&D provides forms for OMR exams and scores completed exams. (OMR is an acronym for Optical Mark Recognition, the technology used to read and score "bubble sheet" exams.) Choose a topic from the list below for more information:
Policies and procedures for exam scoring
Instructors who wish to administer OMR exams are asked to familiarize themselves with the following policies and procedures:
- Answer sheets, instructions for completing the key and answer sheets, and exam scanning forms may be obtained from the Instructional Design & Development (IDD) offices in Belk 115 from 8:00 am to 5:00 pm Monday through Friday. The instruction sheet provides faculty with the information necessary to complete a test key and to have students complete answer sheets properly On the exam scanning form, instructors provide contact information and specify the exam reportsto be printed.
- In general, answer sheets are provided in class sets of 25 each. If a department wishes to procure an entire ream of sheets, please call our office at ext. 5000 or e-mail idd@elon.edu to determine availability.
- Completed exams to be scored should be brought to Instructional Design & Development in Belk 115 between 8:00 am and 5:00 pm Monday through Friday. Our office is closed weekends.
IMPORTANT: Please do not slip exams under our door or send them through the campus mail. We cannot guarantee security when exams are not personally delivered. For security reasons, student workers should neither deliver nor pick up exams.
- When bringing in an exam to be scored, faculty are asked to:
- Sign the exam log sheet.
- Fill out an exam scanning form for each test. (Please request only the reports you need. Sample reports are available in our office.)
- Place the completed exam scanning form in an envelope or folder with the completed answer sheets and test key.
Please note the following:
- The envelope or folder must be large enough to contain the key, answer sheets and the reports that will be printed.
- Please use one envelope for each exam/section. The larger Interdepartmental Delivery envelopes usually work well for this. IDD will not supply envelopes.
- If necessary information is omitted from the exam scanning form, the test key or the answer sheets, the exam will be set aside and you will contacted for the needed information. The most common error is omission or incorrect coding of the required ID number; please ensure that all answer sheets are coded with an ID number.
- Exam scoring is NOT a drop-in service. However, every effort is made to grade exams within one business day.
- You will be called when your exams are ready for pick up. We do NOT deliver or mail scored exams. Note that exams must be picked up by the instructor or an employee who is not a student.
- Note that, upon request, we can e-mail either exam results as a comma-delimited file (i.e., raw data) or specific reports as html files. The scored exam may then be picked up at your earliest convenience.
Available answer sheets
We provide three different answer sheets that are compatible with the NCS ExamSystem II software we use for scoring exams. In most testing situations, either of the first two are recommended.
Form 229633 – Enables students to respond to up to 200 multiple choice (A-E) questions (100 on front, 100 on back of sheet). Also records up to 21 characters for the student's name and up to 10 digits for the student's identification number. This form enables student names to be printed on reports. Consequently, this is the form we usually recommend.
Form 229628 - Enables students to respond to 60 multiple choice (A-E) questions and write an essay on the back of the sheet. Also records up to 21 characters for the student's name and up to 10 digits for the student's identification number. This form enables student names to be printed on reports. Consequently, this is the form we usually recommend when faculty wish to have students complete an essay questions in conjunction with objective test items.
Form 95677 – Enables students to respond to up to 100 multiple choice (A-E) questions and write an essay on the back of the sheet. Also records up to 10 digits for the student's identification number. As this form identifies students by ID number only, not name, we usually do not recommend it.
Important: whichever answer sheet you use, please remind your students to use a #2 pencil and fill in each 'bubble' completely and neatly. Answers that aren't dark enough or that don't fill the circle completely do not scan properly. Erasures should be neat.
Reports that can be generated for exams
The following reports can be generated with the NCS ExamSystem II software we use:
