Students may apply for exemption from the required immunizations on grounds of permanent medical contraindication or conflicting religious beliefs. See the NC Immunization Branch "NC Exemptions" page for more information on medical and religious exemptions.
Students choosing to apply for any exemption must complete the report of health history, physical examination, and TB screening process.
If a physician licensed to practice medicine certifies that a required immunization is or may be detrimental to a person's health due to the presence of one of the contraindications adopted by the Commission for Health Services, the person is not required to receive the specified immunization as long as the contraindication persists.

All requests for medical exemption must be submitted to and approved by the NC Department of Health and Human Services.
A written statement of the student's bona fide religious beliefs and opposition to the immunization requirements must be submitted with signature and date. Please include the student's full name and date of birth.
If an outbreak occurs on campus, unvaccinated students will have to reside off campus and will be excluded from all university facilities until they present valid evidence that they are immunized against the disease, or the epidemic or threat no longer constitutes a significant public health danger.
