Students may apply for exemption from the required immunizations on grounds of permanent medical contraindication or conflicting religious beliefs. See the NC Immunization Branch "NC Exemptions" page for more information.
Students choosing to apply for any exemption must complete the report of health history, physical examination, and TB screening process.
If a physician licensed to practice medicine certifies that a required immunization is or may be detrimental to a person's health due to the presence of one of the contraindications adopted by the Commission, the person is not required to receive the specified immunization as long as the contraindication persists. See the Medical Exemption Request form for details and to request an exemption.
For request for a medical exemption not on the list adopted by the commission, please complete the Physician's Request for Medical Exemption form.
All requests for medical exemption must be submitted to and approved by the NC Department of Health and Human Services.
A written statement of the bona fide religious beliefs and opposition to the immunization requirements must be submitted with signature and date. Please include the student name and date of birth.
If an outbreak occurs on campus, the student will have to reside off campus and will be excluded from all university facilities until they present valid evidence that they are immunized against the disease, or the epidemic or threat no longer constitutes a significant public health danger.
