Print a mail order or a pharmacy claim form, view claims and print benefit explanations on www.myuhc.com? See how it can be done by following the steps provided below.
To print a mail order form or a retail pharmacy claim form:
Save time by printing mail order forms online and mail completed forms along with prescriptions.
- Go to www.myuhc.com and login (or register if it’s your first time visiting the site)
- Click the “Pharmacies & Prescriptions” tab
- Click the “Forms & cards” link and you will see the “Prescription mail order form” and the “claim from for retail pharmacy purchases”
- Print at will!
To view claims and print an explanation of benefits:
Stay on top of your claims information. UnitedHealthcare offers an online Medical Claim Summary that makes it easy for you to see your entire family’s claims and understand how much was coered by your plan and what your out-of-pocket costs are. You can download claims to a spreadsheet, make an appeal, set up automatic payments, direct deposits and more.
- Go to www.myuhc.com and login (or register if it’s your first time visiting the site)
- Click the “Claims & Account” tab or the large “View My Claims” button located on the center of the page
- Click the “Medical Claim Summary” link
- Search or sort by patient, date of service, provider, processed data or status
- Once you find your claim, click on “More Details”
- To print the explanation of benefits, simply click “Download Explanation of Benefits (EOB)”
Note: If you are unable to view a claim on www.myuhc.com, please contact the member services phone number on your ID card.
