Report an Incident or Concern

These reporting forms are for non-emergency reporting only. If any person may be in danger, call 911 immediately. Reports will be reviewed within 1-2 business days of submission. Based on the type of report, you may be contacted for additional information (if you provide your name or contact information). Though anonymous reports are permitted, doing so may limit the University’s ability to investigate and/or respond.

Based on the type of report you submit, appropriate university staff will receive information.

Student Conduct Incident Report

Submit a report of a possible Code of Conduct violation.

Hazing Incident Report

Elon University defines hazing as: "Any activity expected of someone who is joining a group (or who is trying to maintain full status in a group) that humiliates, degrades, or risks harming that person emotionally or physically, regardless of the person’s willingness to participate."

Student Concern Report

Submit a report if you are concerned for a student's health, well being, or behavior. This information, may be used to help link a student with resources and/or respond if a student's behavior is creating a situation that is potentially unsafe for themselves or others.

Identity-based Bias Report

This reporting form is for NON-EMERGENCY incidents of bias and harassment (except when based on perceived or actual sex & gender) that have impacted members of the Elon community. If you are in an emergency situation in which you need medical attention, guidance for reporting or protective orders, or immediate resources, please contact the SAFEline at 336-278-3333. You will be connected with a confidential resource immediately. In the case of sexual violence or intimate partner violence one of Elon University’s Violence Responders will be contacted.

Title IX, Sexual Misconduct, and Interpersonal Violence Report

This reporting form is for NON-EMERGENCY incidents of harassment and discrimination based on actual or perceived sex or gender that have impacted members of the Elon community. This could include incidents of sexual harassment, sexual assault, stalking, relationship violence, etc. If you are in an emergency situation please dial 9-1-1. For guidance for reporting or protective orders, or immediate resources, please contact the SAFEline at 336-278-3333. You will be connected with a confidential resource immediately. In the case of sexual violence or intimate partner violence one of Elon University’s Violence Responders will be contacted.
