Important reminder as you prepare for the fall:

COMPLETED HEALTH FORMS AND IMMUNIZATIONS DUE TO HEALTH SERVICES JULY 1

University policy and State regulations require that all new students submit a health form, physical exam within one year of matriculation, and all required immunizations. If you have not had a physical since July 2012, please make the proper arrangements as soon as possible.

• The University cannot allow you to move into Residence Halls or attend classes (commuters) until all requirements are complete. Please keep a copy of the form for your records.  Copies of all required forms and regulations are available here.

• All Elon University students are required to maintain health insurance either via an employment, parent, spouse or independent policy. You may also participate in the student health insurance policy marketed by Collegiate Risk Management. Information may be obtained by calling 1-800-222-5780 or visit their website. You must provide proof of insurance on your health form.
