The Phoenix Card Office reserves the right to refuse any reader request and to remove equipment during an event. Readers may be reserved for a total of three days per event, Monday thru Friday from 8:00 a.m. – 5:00 p.m. unless otherwise specified. The requesting organization agrees to take full responsibility for the equipment during the date/times listed above in the event the equipment is damaged, lost, or stolen. This form must be completed prior to picking up the equipment to ensure we properly credit your organization after the event is over. All payments made through the readers at an event will be transferred to the account number provided on the request form.
