Please note, if Elon University has not received your deposit, you will not be able to access PhoenixHealth to submit your forms on-line. You may wait until your deposit is received or you may complete the paper documents. Please do not do both.

Covid-19 PCR Test Results

**DO NOT SUBMIT LABCORP PIXEL RESULTS HERE**

All students, faculty, and staff must complete pre-semester COVID-19 testing. If you are unable to use the LabCorp system you may submit another PCR-test.

Do NOT submit antigen test results, which are subject to greater errors.

Do NOT submit a test that you have taken more than 2-3 weeks prior to your arrival on campus.

HOW TO UPLOAD:
  1. Login to PhoenixHealth using your University username and password.
  2. Once signed in, navigate to the upload tab at the top of your patient portal home screen.
  3. Select ‘COVID19PCR’ from the drop-down list and upload your results.

Immunization Record

  1. Login to PhoenixHealth using your University username and password.
  2. Select Immunizations and click on the enter dates dropdown.
  3. You must enter dates for all required immunizations. You may include your entire record for us to keep on file.
  4. In addition to entering the dates in the Immunization section of the portal, you will need paper documentation from your medical provider so Student Health staff can verify your immunization dates. To do so, save a copy of all your records on your device. *Please save the file “Last First imm record” to your desktop and upload it from that location. File names that are too long or file paths that are too long will cause an error message.* From the PhoenixHealth home page, navigate to uploads. Select “Immunization History” from the dropdown. Follow the directions on the page. The student will not be considered compliant until the document is received.

Report of Medical History

Please note – NO physical exam is required.

Tuberculosis Screening Questionnaire

  1. Print out the Tuberculosis screening questionnaire.
  2. Answer each of the questions on the TB screening questionnaire.
  3. If all answers on the screening questionnaire are no, no further testing is required and form should be submitted to Elon Student Health.
  4. If any questions are answered yes, the student will need to consult with his/her primary care provider to complete the TB risk assessment to determine if a TB skin test (TST/PPD) is necessary.  If the provider determines no further testing is necessary, he/she will sign the form indicating no test is needed. Undergraduate students are NOT required to have a two-step TB test.
  5. Save a copy of the document on your device. *Please save the file “Last First TBSQ” to your desktop and upload it from that location. File names that are too long or file paths that are too long will cause an error message.* From the PhoenixHealth homepage, navigate to uploads. Select “TB Screening Questionnaire and Risk Assessment” from the dropdown. Follow the directions on the page. The student will not be considered compliant until the document is received.

Copy of Health Insurance Card (Recommended but not required)

We recommend you copy/scan your health insurance card to be kept on file in Student Health Services. Student Health Services does not file claims with insurance companies, but in the event of an emergency, it may provide Elon University administrators with quicker access to your emergency contact and health insurance policy information (ONLY).

Select “Copy of Student’s Insurance Card” from the dropdown. Follow the directions on the page.

Condition Disclosure Form

Please note that Student Health Services does not release any information related to health history or conditions to any other Elon University office. To make University administrators and/or other offices aware of conditions, please complete the condition disclosure form.
