Records and Form Submission- PhoenixHealth Portal
All records must be submitted via the PhoenixHealth portal. Students will be able to access their the PhoenixHealth portal to submit your forms on-line once their deposit has been received and an Elon e-mail assigned to them. Students who need assistance uploading documents should call Student Health Services at (336) 278-7230 during office hours or by email at healthservices@elon.edu from their Elon email account.

Immunization/Vaccine Record Submission
- Login to PhoenixHealth using your university username and password.
- Select Immunizations and click on the enter dates dropdown.
- You must enter dates for all required immunizations. You may include your entire record for us to keep on file.
- In addition to entering the dates in the immunization section of the portal, you will need to upload paper documentation from your medical provider so Student Health staff can verify your immunization dates.
- To upload your immunization/vaccine documentation (including your COVID-19 vaccine card):
- Save a copy of all your records on your device. *Please save the file “Last First imm record” to your desktop and upload it from that location. File names that are too long or file paths that are too long will cause an error message.*
- Activate your PhoenixHealth portal here.
- From the PhoenixHealth home page navigate to uploads.
- Select “Immunization History” from the dropdown. Follow the directions on the page. The student will not be considered compliant until the document is received and verified by university staff.
Tuberculosis Screening Questionnaire Submission
o Print out the Tuberculosis screening questionnaire.
- Answer each of the questions on the TB screening questionnaire.
- If all answers on the screening questionnaire are no, no further testing is required, and form should be submitted to Elon Student Health.
- If any questions are answered yes, the student must consult with their primary care provider to complete the TB risk assessment to determine if a TB skin test (TST/PPD) is necessary. If the provider determines no further testing is necessary, the provider will sign the form indicating no test is needed. Undergraduate students are NOT required to have a two-step TB test.
o To upload your TB screening documentation:

- Activate your PhoenixHealth portal here
- From the PhoenixHealth home page navigate to uploads.
- Select “Immunization History” from the dropdown. Follow the directions on the page. The student will not be considered compliant until the document is received and verified by university staff.
Health Insurance Information Submission
All students are required to provide health insurance information and provide a copy/scan of your health insurance card to be kept on file in Student Health Services. Student Health Services does not file claims with insurance companies, but in the event of an emergency, it may provide Elon University administrators with quicker access to your emergency contact and health insurance policy information (ONLY).
- To upload your health insurance documentation/card:
- Save a copy of your records on your device. *Please save the file “Last First Insurance Card” to your desktop and upload it from that location. File names that are too long or file paths that are too long will cause an error message.*
- Activate your PhoenixHealth portal here
- From the PhoenixHealth homepage, navigate to uploads.
- Select “Copy of Student’s Insurance Card” from the dropdown. Follow the directions on the page.
Health History and Information Submission
Please note – NO physical exam is required.
