Chair Forms

(Helpful tip to Department Chairs: You may fill in the forms online and then print to sign)

These forms will only be accepted when signed by the acting chair of the correct department. Students must obtain the signed form from the Chair and return to the Registrar's Office in Alamance 102 to insure there are no schedule conflicts with other courses. 

Falsification of either form is a violation of the Academic Honor Code.
