We are in the process of moving our electronic forms to a more secure platform. If you are off campus, the forms that have already been converted (FERPA, Audit, Address Change, Degree Audit Adjustment) require a VPN login. You may access the Cisco VPN installation instructions and a helpful video tutorial on the Knowledge Base.
The forms that are not yet converted, typically work best Google Chrome. If you see a “401 Unauthorized User” or “Critical Error” please try using Google Chrome.
A few forms are still in PDF format and denoted with a PDF icon () in the lists below. After you complete the fillable PDF form, please print, obtain the appropriate signatures then return the completed form to our office in Alamance 102. Please note, some PDF forms may take a moment to load. They work best in Google Chrome or Firefox. If the form does not require additional signatures you may fill out the fields and click File > Send File. This should automatically attach the file to a new email and you may email it to firstname.lastname@example.org.
