Pass/Fail Form

Please contact your advisor and discuss your intent to take a course pass/fail prior to submitting this form to insure timely processing.   Please read all instructions on the form and all follow-up information carefully.  There is a required agreement to register for a class pass/fail.  A course taken pass/fail may not be used for your major, minor or general studies requirements and cannot be change to a grade later.

You must be enrolled in the course prior to requesting to take it pass/fail

This form is designated for students who are officially registered for a course they wish to change the status to pass/fail.  This form will not enroll a student into a course which is not currently listed on their schedule.
